Who Can Apply?

• All patients are welcome to apply.
• Eligibility is based on household income and size only.
• No one will be denied medical care because they cannot pay.

Required Documents

Submit proof of household income within 5 business days, such as:

• Pay stubs
• Tax return or 1099
• Social Security, SSI, SSDI, unemployment, or disability award letters
• Pension or retirement statements
• Child support or alimony documents
• Bank statements
• Other acceptable income verification

• If income verification is not submitted within 5 business days, standard charges will apply.
• Eligibility must be renewed as required and updated if your household income or size changes.
• Discounts apply only to services provided by Redeem Health Care Medical Systems and do not include third-party laboratory services or prescription medications.
